The Situation

Mrs A had been admitted to hospital following a fall in which she had broken her leg. Aged 81 and lacking any close family nearby she contacted her solicitor concerned about what care she would receive and how it would be managed when she was eventually discharged from hospital.

How we helped find care

Following a referal from her solicitor, Clarity Care Consulting arranged for one of their Independent Care Advisors to visit her on a busy ward in the major hospital where she was being treated.

Hospital Visits

When our consultant arrived, Mrs A was very distressed as the hospital discharge team were considering discharging her to a community hospital some distance from her home, which would prevent her friends from being able to visit.

After intervening on her behalf, it was agreed that she would be discharged to a more local hospital where we continued to visit her and liaise with her GP, nursing staff and occupational therapy team about her wishes for her eventual return home. We advocated on her behalf to ensure her views and wishes were taken into account by coming up with creative solutions to meet her care needs.

We worked closely with the occupational therapy team to ensure a package of support and equipment was agreed which Mrs A was happy with. This included visiting her house with the occupational therapists to agree what equipment was to be installed and researching other options where necesary. 

Care At Home

In preparation for her return home, our independent care advisor researched a number of live-in care companies and organised an initial 2-week package to ensure she had sufficient care and support at home to promote both her recovery and her independence.

A further domicilliary care package was arranged to start 2-weeks after her return home which provided daily home care visits and gave that little bit of extra support as she continued her recovery.

Throughout this process we provided regular updates on the proposed solutions, together with costs, to the solciitor so they were always aware of what was being put in place, how this would meet their client’s needs and ensuring it was affordable to their client. 

The Final Solution

After nearly 3 months in hospital Mrs A returned to her own home on her own terms. With our help she was able to have the care she needed when and where she wanted it.

This was a great outcome fboth for Mrs A and her solicitor, as her solicitor was able to focus on her legal affairs knowing that their client was well supported throughout the process.
