Finding Care Services

Our aim is to ensure individuals and their families receive the best care advice and support possible, be that at home, in a residential setting or in hospital.

We provide a personalised service helping to find the right care for you or your loved one by being there at every stage of the process from beginning to end and acting as a main point of contact between families and health and social care professionals.

Care In Your Own Home

Our experienced Care Advisors support older people and their families in finding, arranging, and monitoring care provided at home. This can range from help around the home and a few visits a day to 24 hour live-in care.  Our expert knowledge of the social care system can help ensure the right care is delivered in the right way, when and where it is needed.

We can also help where problems have arisen, or care needs have changed.

Residential and Nursing Care Homes

When an older person can no longer stay in their own home, our experienced Care Advisors help you and your family consider the range of options available and identify a home that would best meet your needs. We understand what a difficult and stressful time this can be and offer services designed to provide support and reassurance throughout the process.

We meet with our clients and their families to discuss their needs and wishes and based on this information carry out research on our clients behalf and provide a shortlist of homes that meet their criteria.

For those families who live at a distance or who are juggling other commitments this takes the stress and worry out of finding the right care for their loved on.

Funding Advice

Clarity Care identify when clients are likely to be eligible for CHC and other NHS funding, Local Authority Funding or state benefits, including Attendance Allowance and help to apply for these funds.

Concluding the Service

After a follow up assessment, Clarity Care will update loved ones or professional attorneys involved and write an update report which can be delivered in person or sent with a letter to conclude the service.
